About the supplement
The care management supplement is available to registered providers delivering ongoing Support at Home services.
This supplement supports eligible participants with specialised needs to access additional care management services.
Eligibility
The care management supplement is available if a Support at Home participant is:
- an older Aboriginal or Torres Strait Islander person
- homeless or are at risk of homelessness
- a care leaver (someone who has spent time in institutional or out-of-home care, including Forgotten Australians, Stolen Generations, and former child migrants)
- a veteran who is approved for the Veteran’s supplement for aged care
- referred to Support at Home from the care finder program.
The supplement is only applied once, even if a participant meets multiple criteria.
Supplement rate
The supplement funds an additional 12 hours of care management activities per year (3 hours per quarter) per eligible participant.

Getting the supplement
Participants are identified by Services Australia from their aged care assessment.
Providers can also complete the Provider Application for Care Management Supplement Form to notify Services Australia of participants who meet the eligibility criteria (if the care management supplement has not already been applied).
The care management supplement is credited to your care management account.
