This notice explains how we (the Australian Government Department of Health and Aged Care) manage personal information included in the Palliative Care Status Form consistent with our obligations under the Privacy Act 1988 (the Privacy Act) and the Australian Privacy Principles (APPs).

Information we collect

We collect the:

  • care recipient's
    • name
    • date of birth
    • gender
    • care recipient ID or Aged Care Gateway ID
    • Medicare card number
    • Department of Veterans' Affairs card number (if applicable)
    • medical assessment – life expectancy
    • Australia-modified Karnofsky Performance Status (AKPS) score.
  • residential aged care service's: 
    • National Approved Provider System ID
    • name
    • address
    • phone number
    • approved provider email address
  • Medical practitioner's or nurse practitioner's:
    • name
    • practice details
    • phone number
    • email address
    • Australian Health Practitioner Regulation Agency ID number
    • Medicare Provider Number.

Why we collect this information

We collect this information to ensure care recipients who enter a residential aged care service to receive end of life care are correctly classified in the funding class that recognises the cost of palliative care, without the need for the care recipient to undergo separate classification assessment. 

The Australian National Aged Care Classification (AN-ACC) funding model includes a separate funding class for individuals who enter residential care for the purpose of receiving palliative care. 

The Palliative Care Status Form collects information used to identify whether a new care recipient should be classified to receive palliative care funding. 

How we collect this information

We collect this information from the Palliative Care Status Form.

Who will be able to see this information

Only authorised Department of Health and Aged Care staff can access personal information stored by us. Security features protect it from unauthorised access.

The care recipient who is the subject of the information will be able to view resulting classification status through the My Aged Care Gateway.

How we store your information

We hold your personal information in the My Aged Care system, which is hosted on secure databases in Australia.
