Support at Home transition activities

Find out what providers still need to do in the first few months to help ensure a smooth transition to the Support at Home program.

On 1 November 2025, Support at Home replaced the Home Care Packages Program and the Short-Term Restorative Care (STRC) Programme. As a Support at Home provider, there are final steps you need to take to ensure a smooth transition to the new program.

Publish your prices 

By 7 November, you must publish pricing information for each Support at Home service you deliver on your own website.  

This information needs to specify the price for services during: 

  • standard weekday business hours (for example, 8am to 6pm)
  • non-standard weekday hours (Saturdays, Sundays and public holidays).

Verify services and pricing 

By 7 November, you must use the My Aged Care Service and Support Portal to: 

  • verify and update the services you deliver – this includes removing services that are not included in the Support at Home service list
  • update pricing for the services you deliver
  • revalidate that your service delivery area is accurate
  • confirm outlet names and descriptions.

If you miss this deadline, your service and pricing details won’t show correctly in the My Aged Care’s Find a Provider tool.  

Going forward, please keep your details up to date and notify us about certain changes.

Finalise Home Care Package claims

By 30 November, you are strongly advised to finalise all outstanding claims for Home Care Package (HCP) services delivered to 31 October via the Aged Care Provider Portal.

If applicable, you must report your balance of provider-held HCP Commonwealth unspent funds in your final claim.

If you miss this date, it will impact your ability to make timely claims for Support at Home services from 1 December. 

From December, you will be able to claim as needed (up to once daily).

Update common price for each service

Between 1 and 30 January, you must update your common price for each service.

You need to:

  • calculate your common prices over the previous 2-month reporting period
  • review your prices every 2 months to align with specific reporting periods
  • update any change in common prices in the My Aged Care Service and Support Portal within 30 days.

AT-HM scheme data collection

You have until February 2026 to undertake the Assistive Technology and Home Modifications (AT-HM) scheme data collection.

You will need to collect and submit information on transitioned participants who need assistive technology and home modification, beyond their HCP unspent funds.

Implement program guidance

You must deliver Support at Home services in line with program guidance and legislative requirements in the new Aged Care Act.

Please refer to the program manual, provider training and other Support at Home resources to understand and comply with the rules, procedures and responsibilities for the new program.
