About the AT-HM scheme data collection
The AT-HM scheme data collection allows transitioned HCP care recipients to access AT-HM funding without a Support Plan Review. This provider-led process is open until February 2026.

Prepare to collect data
To prepare for collecting data, you can:
- read the guidance material and download the data collection form
- have conversations with your Home Care Package care recipients to understand their assistive technology and home modifications needs
- prepare evidence, such as invoices, quotes or prescriptions, to support the funding needs for each older person
- appoint a coordinator within your organisation to register for the Health Data Portal.
Collecting data
When collecting your data:
- you can organise bulk requests in multiple submissions for AT-HM funding tiers for transitioned HCP care recipients
- each care recipient can only be included in the data collection once
- please submit requests promptly
- submit the data collection form via the Health Data Portal
- attach supporting evidence to the client records in the My Aged Care Service and Support Portal (ensure care partners upload evidence promptly to avoid processing delays).
Note: Attaching supporting evidence is mandatory for collection processes, consistent with requirements for submitting AT-HM evidence (e.g. invoices and prescriptions) for Support Plan Reviews for changing AT-HM funding tiers.
Once processed
Once you have confirmation that the request has been finalised for the participant, your next step is to submit an Entry Notification through the Aged Care Provider Portal (Services Australia). This notification informs Services Australia that the participant has entered care.
