About us
Our Local Network is the face of the department in the community, with offices in every capital city and 9 regional sites.
We are on the ground, engaging and building relationships with people in the health and aged care sectors to:
- understand the specific local health and aged care needs
- identify and address local issues and gaps
- support provider capability
- tailor solutions to meet the needs of older people in each region.
Why it is important
We work in our regions with a focus on aged care, primary care, and First Nation’s health.
This means identifying local issues so that national policy considers and responds to the needs of different communities.
What we do
We support a health and aged care system that works for everyone by:
- building local knowledge and strong, collaborative relationships across the local health and aged care sectors
- collecting, analysing and sharing local data to identify current or potential service gaps
- collaborating with health and aged care providers to
- ensure continuity of quality care
- enable appropriate care pathways from hospitals
- support emergency responses
- building capacity and raising awareness by
- helping providers understand and implement aged care reforms
- talking to older people and their families in the community
- supporting services affected by emergencies including disease outbreaks and natural disasters.
