Reporting for MPS providers

As an MPS provider you must send us:

  • an annual activity report
  • de-identified client files
  • an annual statement of financial compliance and income and expenditure.

These are due by 31 October each year.

This report should cover all activities in your service delivery plan. We will provide you with a template each year.
