Vaccine Administration Partners Program – Request for quotation template

Immunisation providers (IProviders) in receipt of a Request for Quote (RFQ) wanting to respond must complete and return the RFQ to the requesting Agency as a quotation for the provision of services.

Vaccine Administration Partners Program – Enterprise order request

Immunisation providers (IProviders) must provide Enterprise Order Request to the department. The Enterprise Order Request will become an Enterprise Order when the department approves it.

Vaccine Administration Partners Program – Price schedule template

Immunisation providers (IProviders) must complete the pricing schedule and send it to the department as a quote or estimate cost of providing a service through their allocations.

Vaccine Administration Partners Program – Consolidated scheduling template

Immunisation providers (IProviders) must complete the scheduling template and send it to the department based on accepted allocations. This report provides the department with visibility and confirmation of vaccines required and residential aged care or disability care facilities clinic dates.

Vaccine Administration Partners Program – End of day reporting template

Immunisation providers (IProviders) must complete the end of day report and send it to the department as a progressive report based on clinic data and representing doses administered to either residential aged care or disability care facilities.

Panel Head Agreement

IProviders who have been successfully appointed to the panel, must enter into a Panel Head Agreement — a deed between the Commonwealth of Australia, as represented by the Department of Health, and the IProvider for the National Vaccine Administration Partners Program.
