Assistive Technology and Home Modifications (AT-HM) scheme data collection

The AT-HM scheme data collection allows transitioned Home Care Package (HCP) care recipients with low or no unspent funds to access AT-HM funding without a Support Plan Review. This is a provider-led process running until February 2026.

Prepare to collect data

To prepare for collecting data, you can:

  • read the guidance material and download the data collection form
  • have conversations with your Home Care Package care recipients to understand their assistive technology and home modifications needs
  • prepare evidence, such as invoices, quotes or prescriptions, to support the funding needs for each older person
  • appoint a coordinator within your organisation to register for the Health Data Portal.

Collecting data

When collecting your data: 

  • you can organise bulk requests in multiple submissions for AT-HM funding tiers for transitioned HCP care recipients
  • each care recipient can only be included in the data collection once
  • please submit requests promptly
  • submit the data collection form via the Health Data Portal
  • attach supporting evidence to the client records in the My Aged Care Service and Support Portal (ensure care partners upload evidence promptly to avoid processing delays).

Note: Attaching supporting evidence is mandatory for collection processes, consistent with requirements for submitting AT-HM evidence (e.g. invoices and prescriptions) for Support Plan Reviews for changing AT-HM funding tiers.
