All approved providers must tell us when certain things happen, including:
- providing new services
- changes to existing services
- care recipient starting and ending home care.
New service
For each new service, tell us the name, address and contact using the Home Care Service Notification Form.
You must do this before you start providing in-home aged care through this service.
Changes to services
Use the Home Care Service Notification Form to tell us about changes to your existing service’s name, address or contact details.
You must do this within 28 days of the change.
Care recipient starting home care
Within 28 days of a care recipient starting home care with you, you must tell Services Australia through the Aged Care Provider Portal.
Care recipient ending home care
Within 31 days of the day you stop providing care to a care recipient, you must submit the care recipient's name and cessation date through the Aged Care Provider Portal.

Notifying the Aged Care Quality and Safety Commission
You must also tell the Aged Care Quality and Safety Commission (ACQSC) within 14 days of when there are changes:
- that affect provider suitability
- to key personnel or their suitability.
You must keep a record of all notifications to the ACQSC.
