Record keeping
You must keep records about:
- participants, including eligibility for supplements and care notes
- allegations or suspicions of reportable incidents
- service staff influenza vaccinations
- staff members and volunteers police certificate checks
- service staff.

Quarterly Financial Report
You must submit the Quarterly Financial Report (QFR) 4 times throughout the financial year to report on:
- financial viability and prudential compliance questions
- year-to-date financial statements, at the registered provider level
- details of direct care labour costs.
Legislated submission dates for the 2025-26 financial year are:

Aged Care Financial Report
You must submit an Aged Care Financial Report (ACFR) by 31 October each year.
You should report on 1 July to 30 June period:
- income from services provided, subsidies and supplements
- expenses including wages, administration and non-care related items
- other financial information including assets, liabilities and equity.

Provider Operations Collection Form
You must submit the Provider Operations Collection Form to report information about annual operations by 31 October each year.
You should report on 1 July to 30 June period, including:
- executive position details
- governing body membership
- signed statement of provider compliance with legislative requirements
- diversity information
- common feedback and complaints received by each service
- key improvements made to service quality.
