Transitioning to Support at Home

The Support at Home program started on 1 November 2025. This section explains what providers still need to do to transition to the new program, such as training workers and helping transitioned participants.

  • Transition activities
    Find out what providers still need to do in the first few months to help ensure a smooth transition to the Support at Home program.
  • Assistive Technology and Home Modification (AT-HM) scheme data collection
    The AT-HM scheme data collection will allow transitioned Home Care Package care recipients with low or no unspent funds to access AT-HM funding without a Support Plan Review. This is a provider-led process running until February 2026. 
  • Provider training
    This self-paced training helps registered providers and their workers understand their responsibilities to deliver the new Support at Home program.
