Monthly statements for Support at Home services

You must give a monthly statement to each participant to show their available funds, how the funds are being spent and any unspent amounts. Find out what to include and when to provide it.

What is a monthly statement

A participant’s monthly statement is a document that shows, for the previous calendar month: 

  • the total government funding that was available to them
  • any participant contributions
  • the services you provided and the price charged for each of those services
  • the amount of any unused funding
  • the closing balance of their budget at the end of the month.

What to include

You must include: 

Care management

For participants accessing ongoing services, you must include the total units or hours of care management delivered that month.  

For participants in the Restorative Care Pathway or End-of-Life Pathway, you must include the:

  • total unit or hours of care management delivered that month
  • price
  • amount of government subsidy you received.

Template

You may wish to use our monthly statement template. This can:  

  • make statements easier for participants to understand
  • help participants make informed choices about the use of their funds. 

This template is a guide only. You can choose to use your own template, but it must include the required information.

Support at Home monthly statement template

This document is an example of the monthly statement that providers will send to their participants under the Support at Home program. Providers’ templates may look different.

Preparing monthly statements

You must: 

  • prepare the statement in plain language so that it is easy to understand
  • help the participant to understand how their budget works and what funds are available.

Monthly statement guide for older people

This is a guidance document intended to help participants better understand their monthly statement under the Support at Home program. Providers’ monthly statements may look different.

When and how to provide

You must provide a statement to a participant each month, even if you did not provide Support at Home services to them that month.

Give it to the participant or their registered supporter (or both). You can provide a hard copy or an electronic copy.

You must do this by the last day of the following month.

For example, the deadline to provide a July monthly statement is 31 August.
