Delivering services for Support at Home

As a registered provider, you deliver care and services to meet Support at Home participants’ assessed needs. This section outlines what services you can deliver, care management and the short-term pathways.

  • Services 
    You can offer a range of ongoing and short-term services through the Support at Home program. These services should support each participant’s assessed aged care needs, goals and preferences.
  • Care management 
    You must undertake care management activities for all participants. Dedicated care partners must work with participants to plan and review their care and services.
  • Self-management
    Participants can choose to manage parts of their care under Support at Home. You are still responsible for the care and services delivered to participants, including care management.
  • Assistive Technology and Home Modifications (AT-HM) scheme
    This scheme provides separate funding to participants for the assistive technology and home modifications that they need, which is available for 12 months (in most cases).
  • Restorative Care Pathway
    This pathway provides up to 16 weeks of intensive allied health and nursing services to restore a participant’s function, support ongoing independence, regain skills and prevent further age-related decline.
  • End-of-Life Pathway
    This pathway supports older people with 3 months or less to live who want to remain at home. It offers extra in-home aged care support for 12 weeks and is designed to complement state and territory palliative care services.
  • Interaction with other programs
    Support at Home interacts with other government-funded aged care programs. In some cases, participants may be able to access other programs at the same time as Support at Home.
