This manual is for Assessment Organisations. It guides assessors on using good practices in the assessment of older people’s support needs and their eligibility for Australian Government–subsidised aged care services.

Assigning Priority Level for Service fact sheet

This fact sheet helps assessors understand priority levels when assessing clients for services under the Commonwealth Home Support Programme or Home Care Packages. It also explains when a client’s priority needs to change.

Guidance for Home Care Package High Priority

This guidance is for clinical aged care needs assessors. It informs and supports their decision making when recommending a high priority for a Home Care Package.

Integrated Assessment Tool (IAT) Offline Form

The IAT Offline Form is to be used by assessment organisations. This form is provided as an alternative way to record assessment outcomes when IT is not available or
appropriate for client engagement, and as a tool to support training.

Aged Care Assessor App

Aged care assessors can use this app to conduct assessments (using the Integrated Assessment Tool or Australian National Aged Care Classification Assessment Tool). If there is no internet, first download client information from the assessor portal and upload the assessment when back in the office.

Changes to publication of room pricing on My Aged Care

On 1 July 2024 we are updating the My Aged Care Service and Support Portal. This will change how residential care providers enter room pricing information for publication on the My Aged Care website.
