This form is for older Australians who want to apply for Australian Government-subsidised aged care, and for approved providers seeking approval for an emergency case.
Approved providers should submit it to the assessment organisation within 5 business days after the day care starts.
The form is also available in the assessor and service provider portal on the forms page of the ‘reports and documents’ tab.
