Whatever your age may be, you will have values and preferences related to your health and personal care. It’s important to think about these values and preferences and write them down. You can then let your loved ones know of your wishes should you be living with a life-limiting illness.

- how to get palliative care
- who would you like to provide the care?
- what kinds of treatment can you or can’t you live with?
If you haven’t thought about your end-of-life care options, you can start by thinking about things like:
- what do you want for your end-of-life care?
- would your loved ones know what you want for your end-of-life care?
- who would make decisions about your end-of-life care if you couldn’t?
- where would you like your care to be (home, hospital or hospice)?
- who would you want with you when you die?
Once you have thought about what you want, you can start the conversation with your loved ones.
