My Aged Care has resources that the aged care sector can order to share with their clients. This includes brochures, booklets and checklists. These resources are also available in up to 22 other languages.

Aged Care Act 2024 Statement of Rights – A3 poster

This is a double-sided A3 poster which summarises what rights older people have when accessing aged care services. Providers can pin it up around their organisation, such as resident common areas, staff break rooms and bulletin boards.

AT-HM scheme guidelines

These resources explain the AT-HM scheme and how it operates under the Support at Home program. The appendices include information for providers on prescriptions and claiming.

Summary of indicative Support at Home prices

This fact sheet includes indicative price ranges for most Support at Home services. These price ranges reflect what Home Care Package Program providers have reported that they intend to charge for services under Support at Home.

AT-HM scheme data collection

These resources provide guidance on submitting funding requests for the assistive technology and home modifications needs of their Home Care Packages (HCP) care recipients transitioning to Support at Home. They are for staff of approved organisations providing home care services.
