Communication case study – Luca – Transcript
Luca, hi.
Come on in and take a seat.
Hello, Luca.
Phil, good day.
Hi.
I'm Cameron Lucas, support worker.
Hi, Cameron.
Come on.
What seems to be the trouble, Luca?
And what's what's the issue?
Luca's been unwell.
A bit of a fever and a cough.
I see.
OK, let's have a look, shall we?
coughing noises
No.
No.
Go away.
Luca.
Luca, it's OK.
The doctor was just trying to examine you.
All right?
Luca likes to know what's going to happen.
It would be good if next time you could give him some warning and talk him through what's going to happen.
Yes, of course.
Sorry, Luca.
Sorry about that.
Come on, let's sit down.
Sorry about that, Luca.
How about we get to know each other a little bit better first?
Yeah.
Do you do you like sports?
Are you, are you into sport?
Soccer.
You like soccer?
Yeah, I like football too.
I like football.
Dancing
And you like dancing as well?
Tap, tap, tap dancing.
Oh, tap dancing.
Wow.
That's that's very aerobic to do that sort of thing.
We need to be really healthy, don't we?
Yeah.
Yeah.
Be healthy.
Yeah.
Well, if we're gonna do things like that and we need to be healthier, how about how about I listen to your chest so that we can work out if we can fix this cough?
Is that, you're on board with that?
Yes.
Yeah.
Yeah.
OK.
In order to listen to your chest, I'm.
I'm going to need to place this here.
The doctor's just going to need to lift your shirt.
OK.
OK, then yes, OK, Luca, I'm, I'm going to I'm going to listen like this.
And I need to place this on your chest.
So, so can you lift your shirt up?
There you go.
Here you go.
Thank you.
So I think that's that cough's going to clear up quite nicely, but it will mean that you need to drink lots of fluids, so drinking lots of water, and you're going to need to take some medicine as well.
So I'm going to suggest you take paracetamol every four to six hours.
So that means you'll take it four or five times a day.
OK Cameron, can I get you to remind him?
Yeah.
And and then you'll need to come back in in two weeks and we'll do a check up just to see how things are tracking.
All right.
You're also due for a full health check, so we'll probably do that while we while we've got you in, but it's quite a lengthy procedure, so maybe we could split it into two appointments.
I think that's better.
I'll see.
You'll see.
OK.
Are you happy with that?
Yeah.
Great.
Wonderful.
Now, before the next appointment, I was just wondering is there anything that I could do that would make it easier for you next time, Luca?
Like, is there a way that I could somehow make it easier for you to understand the sorts of things I'm saying And for you.
Yeah.
How about your pictures?
Pictures.
Yeah.
Pictures.
Luca likes to use pictures.
I see.
OK.
Oh, that's great.
We can use pictures.
Maybe you could get Cameron to send me a few examples of the pictures that you like to use.
Of course I can.
Great.
Thank you very much, Luca.
It was really good seeing you both.
And I'll see you again in two weeks.
OK.
OK.
Oh, you're going to go back door.
Yes, that's right.
This way.
Thank you.
Dear Doctor Benson, I've attached those picture boards.
Luca uses these as aid for communication.
I hope they come in handy for the next visit.
All the best, Cameron.
Luca, come on through.
Good to see you again.
Cameron.
Hi.
Thank you for coming in and thank you for sending through the picture cards.
As you can see, we've got them printed off.
I can hear that your cough is gone.
That's really excellent.
Happy.
Happy, Excellent.
Well, this is a good place to start.
I'll get you to indicate how you're feeling today.
Happy.
Happy.
Right up here.
Happy.
Excellent.
Well, I'm glad you're happy and that your cough has cleared up.
As I mentioned last time, today's also going to be a full health check.
We're going to measure your temperature, pressure, your blood pressure.
Oh, blood pressure.
And like last time, we're going to listen to your heart.
Add to the heart.
That's right.
So that is that OK with you?
Yes.
Oh, Oh, great.
Excellent.
OK, I'm also going to feel your tummy and then I will need to look at your eyes, your ears and your throat.
Do you understand?
Yeah.
Yes.
OK, good.
So I would recommend that you get the blood tests done relatively soon.
OK, Luca.
Oh, and we have the healthcare plan for you here now.
Now this outlines what you need to do from here.
It's important to understand that we're here and we care about your health.
As you can see.
If you have questions, always ask.
Yeah, yeah.
Do you understand that?
And this is very important.
Luca, I might ask you a question, but we don't do anything without you saying yes, yes, yes.
That's right.
Sometimes we might give you some medicine and you get to say no or yes to anything.
I say yes.
Great.
OK.
Right now it's been very good to see you both again.
But before you head off, Luca, is there any more questions that you have for me?
OK, Thanks.
Thank you.
Thank you.
Yes.
Now I'm really grateful to see you.
I just want to make sure that if you've got any questions, now's the best time to ask Carl back.
Come back.
When will you come back?
Yes, over here in three months.
You'll see you come back and we'll have a review in three months.
It's all here in the healthcare plan.
See you when I come back
Yes.
And you'll see me again when you come back in three months.
It's all written down here in the healthcare plan.
Would you like to hold on to those or Cameron, would you like to hold on to them?
Thank you.
I say thank you.
Thanks.
Thank you.
It's a pleasure.
Thank you.
Thank you for coming in.
And I'll see you in three months.
See ya.
Bye.
See ya.
Bye.
Bye.
Bye.
Thanks, Cameron.
This case study video is one of two films (4-8 mins long) that aim to help students to better understand how they can work with people with intellectual disability and their supporters. The films focus on communication, and decision-making and consent when working with people with intellectual disability. The case study films were developed as part of a suite of education resources designed to support the Intellectual Disability Health Capability Framework. This case study film focuses on communication.
The script for this case study film was co-designed and stars an actor with intellectual disability. The film starts by showing a poor interaction between a person with intellectual disability and a health professional. The film then goes on to show ways that health professionals can improve their communication and use of reasonable adjustments to better support people with intellectual disability.
