Support at Home program manual (version 3) – A guide for registered providers

The Support at Home program manual is a guide designed to assist registered providers to understand and comply with the Support at Home program rules, procedures and obligations.

This manual assists registered providers to understand their responsibilities and the delivery of care under the Support at Home program. It includes how the Support at Home program operates, how to work with participants to develop a care plan and a budget, information on participant contributions and the short-term pathways for AT-HM, restorative care and end-of-life services.

We will make further updates to the program manual once the Aged Care Rules are finalised.
