The Single Employer Model (SEM) trials aim to improve the recruitment and retention of General Practitioners (GPs) and Rural Generalists (RGs) in regional, rural, and remote areas by allowing registrars to be employed by a single state or territory health service throughout their training. This model simplifies employment conditions and enhances access to entitlements like leave.
Key components include:
- s19(2) Direction: An exemption under the Health Insurance Act 1973 that allows SEM-employed registrars to bill Medicare while working in private practices. It ensures no double-dipping of government funds and outlines eligible Medicare Benefits Schedule (MBS) items and in Modified Monash Model locations (MMM), 2–7.
- Memorandum of Understanding (MoU): Required between the Commonwealth and jurisdictions, outlining roles, responsibilities, and trial parameters. Up to 80 registrars per jurisdiction can participate.
- Roles and Responsibilities:
- Commonwealth: Oversees MoUs, issues s19(2) Directions, and funds training and evaluation.
- State/Territory Health Services: Employ registrars, manage trials, and ensure data reporting and stakeholder engagement.
- GP Colleges: Maintain training standards and support registrar placements.
- Practices: Provide training environments and enter agreements with health services.
- Trial Duration: Runs until 31 December 2028, with a national evaluation informing future decisions.
- Key Principles: Voluntary participation, financial neutrality for practices, collaborative relationships, and flexibility in model adaptation.
