Patient privacy and consent statement – NORDR

This document contains the National Occupational Respiratory Disease Registry patient privacy and consent statement. It sets out the step by step process for a physician to understand when patient consent is and is not required to notify information to the Registry.

The document also contains the patient privacy statement and contains patient consent forms for the patient’s physician to pass to the patient to obtain their written consent to their information going into the Registry.
