This report provides the final evaluation of the Aged Care System Navigator (ACSN) trials. The ACSN measure is aimed at testing different navigation models to help people understand and engage with the aged care system for people who have had difficulty in accessing the services they need.

The evaluation was undertaken by Australian Healthcare Associates (AHA) and is based on its analyses of data collected from the trials between 28 February 2019 and 5 February 2021.  The evaluation’s Final Report includes updates to the findings that were provided in the Interim Report’s Executive Summary (published 9 October 2020).

The final evaluation:

  • looks at the outcomes of the trials
  • reviews other system navigator models
  • reports on stakeholder views
  • identifies important principles, service delivery elements and implementation considerations for future aged care navigation services.
