Agent – Personal information collection notice – NORDR

This document contains the National Occupational Respiratory Disease Registry agent personal information collection notice and a consent form.  This notice sets out how the department will handle the personal information of an agent that is acting on behalf of a physician to access the National Registry and provides a consent form.
