Upcoming changes to bulk billing incentives in general practice

There are 2 parts to the upcoming changes to bulk billing incentives in general practice, from 1 November 2025:

  1. Eligibility for Medicare Benefits Schedule (MBS) bulk billing incentives (BBIs) will be expanded to all Medicare-eligible patients (currently only children under 16 and Commonwealth concession card holders are eligible). BBIs are additional MBS payments made when a doctor bulk bills an eligible patient for an eligible service. The value of MBS BBIs depends on the service provided and the location.   2. The Bulk Billing Practice Incentive Program (BBPIP) will be introduced.

The Bulk Billing Practice Incentive Program

From 1 November 2025, the Australian Government will support thousands of general practices to bulk bill every patient, with the launch of BBPIP. 

Practices participating in BBPIP will receive an additional quarterly 12.5% incentive payment on MBS benefits paid from eligible services, split evenly between the GP and the practice. The even split of the BBPIP incentive payment recognises the role of both practices and GPs in delivering bulk billed services to patients.

Participating practices must bulk bill every eligible service for every Medicare-eligible patient, to receive the BBPIP incentive payment. This incentive payment will be in addition to Medicare benefits and MBS BBIs.

Why it is important

Expanding eligibility for MBS BBIs to all Medicare-eligible Australians and introducing BBPIP will support doctors to continue to bulk bill, improving access to general practice care for all Medicare-eligible Australians.

BBPIP will also support GPs and practices to grow their teams, upgrade their facilities and expand their services.

The combined investment means around 4,800 practices will be in a better financial position if they adopt full bulk billing – meaning they will receive more in Medicare payments than they currently receive in patient fees.

How to register for BBPIP

Registration for BBPIP will be open from 1 November 2025. Practices will need to register for BBPIP via Services Australia’s Organisation Register. Further details and instructions on how to register in BBPIP will be provided later this year. 

To participate in BBPIP, practices will need to:

  • Bulk bill all BBPIP MBS eligible services
  • Be MyMedicare registered:
    • National General Practice Accreditation Scheme (NGPA) accredited practices can prepare for BBPIP registration in advance, by registering for MyMedicare now. Instructions on how to register for MyMedicare are available on the Services Australia Health Professional Education Resources website.
    • If you are not NGPA accredited, you should wait to register for MyMedicare and participate in BBPIP from 1 November 2025, when you will be exempt from MyMedicare accreditation requirements.
