Bulk billing incentives in general practice

The Australian Government is investing $7.9 billion to expand eligibility for MBS bulk billing incentives and to establish the Bulk Billing Practice Incentive Program. These measures support general practices to bulk bill all Medicare-eligible patients for eligible services from 1 November 2025.

Bulk billing incentives in general practice

The Australian Government’s investment to support bulk billing in general practice has two components:

  1. Eligibility for Medicare Benefits Schedule (MBS) bulk billing incentives (BBIs) has been expanded to all Medicare-eligible patients. BBIs are additional MBS payments made when a doctor bulk bills an eligible patient for an eligible service. The value of MBS BBIs depends on the service provided and the Modified Monash Model location classification system and MBS BBIs at MBS Online.
  2. The Bulk Billing Practice Incentive Program (BBPIP).

The Bulk Billing Practice Incentive Program

The Bulk Billing Practice Incentive Program supports general practices to bulk bill all Medicare-eligible patients for all eligible services.  

Practices participating in BBPIP receive an additional quarterly 12.5% incentive payment on MBS benefits paid from eligible services, split evenly between the GP and the practice. The even split of the BBPIP incentive payment recognises the role of both practices and GPs in delivering bulk billed services to patients. 

Participating practices must bulk bill every eligible service for every Medicare-eligible patient to receive the BBPIP incentive payment. This incentive payment is in addition to Medicare benefits and MBS BBIs. 

Why it is important

Expanding eligibility for MBS BBIs to all Medicare-eligible patients and BBPIP support for doctors to bulk bill will improve access to general practice care and reduce out-of-pocket-costs for all Medicare-eligible patients.

The combined investment means around 4,800 practices will be in a better financial position if they fully bulk bill – meaning they will receive more in Medicare payments than they currently receive in patient fees.
