22-26 March 2021 is National Advance Care Planning Week

Advance Care Planning Australia encourages everyone to think about and plan their future health care.

Advance care planning promotes care according to your wishes, values, beliefs and preferences. It prepares you and others to plan for future health care, at a time when you may not be able to express those decisions yourself.

Advance care planning benefits everyone: you, your family, carers and health professionals. It helps to ensure you receive the care you actually want.

Advance care planning is a team effort. Start a conversation with your family, friends, doctors, and other health professionals. Having these conversations will help you start thinking about what's important to you.

Visit National Advance Care Planning Week for more information or to find an event in your state or territory. Forms and fact sheets for your state and territory are also available.
