Your personal information is being collected by Ernst and Young. Ernst and Young is contracted by the Department of Health and Aged Care to review and assess complaints about the PHN Program.

We collect personal information for the purpose of reviewing and assessing complaints about the Primary Health Networks (PHN) Program.
What would happen if we did not collect your personal information?
If you do not or are unable to provide your personal information the department and Ernst and Young will consider the issues raised, however may be unable to fully investigate the complaint.
