About the initiative
The Chronic Conditions Prevention and Integrated Care Grants Program is an open and competitive grants program that will fund initiatives to improve health outcomes for people living with, or at risk of developing, chronic conditions.
The program aligns investment with the National Strategic Framework for Chronic Conditions 2026–35. It supports activities that strengthen prevention, early detection, integrated and person-centred care, and health equity.
Why it is important
Chronic conditions are a leading cause of illness, disability and premature death in Australia.
Three out of five Australians have at least one chronic condition, and nearly two out of five have two or more.
The new program aims to improve outcomes for the 15.4 million Australians living with chronic conditions.
These initiatives reflect the Australian Government’s strong commitment to better health for Australians affected by chronic conditions.

Goals
Guided by the strategic framework, the program will invest strategically and competitively to ensure transparency, fairness and alignment with government priorities.
The objectives, aligned to the framework, are:
- health promotion and education
- embedding prevention and early detection
- system-integrated, multidisciplinary, person-centred care
- managing multimorbidity
- enhanced and targeted health equity support.
Meeting our goals
The government has committed $109.9 million over three years from 2026–27 (and $38.3 million per year ongoing from 2029-30) to support the program.
Funding will be allocated to initiatives that show measurable, sustainable improvements in chronic condition outcomes.
There will be different avenues for funding:
- Tier 1 – larger, multi-year, national activities, often delivered by consortiums or partnerships, addressing multiple chronic conditions or shared risk factors.
- Tier 2 – smaller, time-limited activities for individual organisations to deliver targeted projects or pilot initiatives.
Who we work with
We work with successful applicants to support better prevention and care for chronic conditions.
Applicants may apply on their own or as part of a partnership or consortium, in line with the Grant Opportunity Guidelines.
Apply for funding
Applications for the first funding round will open in mid-2026.
When open, the opportunity will be available on GrantConnect.

The guidelines are subject to change. A final version will be published when the grant opportunity is released.
Timing
The first grant opportunity is expected to open in mid-2026, with funding and activities anticipated to start from December 2026.
Noting there is ongoing funding, we will hold another grant round in 2030 for Tier 1 and 2028 for Tier 2.
