An annual health assessment is a comprehensive and structured assessment of a patient. There is strong evidence that these assessments are an effective way to identify unmet health needs in people with intellectual disability.
The Comprehensive Health Assessment Program (CHAP) is an evidence-based tool for carrying out annual health assessments for people with intellectual disability in Australia. This version of the CHAP is for people aged 12 to 18 years.
There is a separate version of the CHAP for adults, people aged 18 years and older.
We review and update the CHAP every year to align with current evidence and clinical guidelines.
