Registered SSBA facilities should use this form to:
- perform confirmatory testing on an SSBA you are not registered for and are reporting the results (positive or negative)
- transfer a suspected SSBA, that you are not currently registered to handle, for confirmatory testing
- destroy a suspected SSBA that you are not currently registered to handle before confirmatory testing
- report an unsuccessful transfer or loss of a suspected SSBA.
