SSBA – Form – Registered facility report for suspected SSBAs and confirmatory results

Facilities registered to handle security sensitive biological agents (SSBAs) should use this form to report specific information about SSBAs.

Registered SSBA facilities should use this form to:

  • perform confirmatory testing on an SSBA you are not registered for and are reporting the results (positive or negative)
  • transfer a suspected SSBA, that you are not currently registered to handle, for confirmatory testing
  • destroy a suspected SSBA that you are not currently registered to handle before confirmatory testing
  • report an unsuccessful transfer or loss of a suspected SSBA.
