SSBA – Form – Non-registered facility report for suspected SSBAs and confirmatory testing results

Facilities not registered to handle security sensitive biological agents (SSBAs) should use this form to report specific information about SSBAs.

Non-registered SSBA facilities should use this form to:

  • transfer a suspected SSBA for confirmatory testing
  • destroy a suspected SSBA prior to confirmatory testing
  • confirm the presence of an SSBA by doing your own confirmatory testing on a suspected SSBA
  • report the results of a test on a suspected SSBA that was sent to another facility for confirmatory testing.
