The report identifies the program has met its short-term outcomes and is on track to meet its medium- and long-term outcomes and has strong support from care finders, Primary Health Networks (PHNs), intermediaries/referrers and clients.
As of 31 December 2024, the program has reported:
- more than 40,000 new client cases
- 70% of client cases involve 5 or more hours of support
- about 30% of cases required more than 15 hours of support, a reflection of the program’s target population, who typically have high/complex support needs.
- Surveyed clients reported positive experiences with the program, with 95% satisfied with the outcome of the care finder process.
Report findings also highlight some issues impacting the care finders role, resulting in care finders having to support clients for longer.
The report presents recommendations aligned to 9 ‘lessons learned’, for both for the department and PHNs.
We will work together with PHNs to consider and, where appropriate, implement these recommendations.
