Public Health and Chronic Disease Grant Program guidelines

The Public Health and Chronic Disease Grant program started on 1 July 2016. These guidelines outline the types of activities that this program funds, and how they are administered.

The Population Health and Sport Division oversee this program, which provides funding across 6 themes:

  • public health and chronic disease procurement
  • cancer control
  • chronic conditions prevention and management
  • family planning
  • national palliative care projects
  • population health improvements

This grant program consolidated funding and activities from 4 other funds:

  • Health Surveillance Fund
  • Chronic Disease Prevention and Service Improvement Fund
  • Health System Capacity Development Fund
  • Health and Social Surveys Fund
