National Strategic Framework for Chronic Conditions

The National Strategic Framework for Chronic Conditions 2026–2035 sets Australia’s long term plan to prevent and manage chronic illness. It guides governments, health services and communities to work together, focusing on prevention, integrated care, equity and shared risk factors.

The framework will:

  • align action across governments, health systems, service providers and communities by creating a shared national policy foundation
  • apply across the whole-of-life, from prevention through to long-term management
  • prioritise multidisciplinary care and continuity of care, not just ‘joined-up care’
  • focus on managing multimorbidity – people with multiple chronic conditions
  • advance health equity through targeted support for priority populations
  • support system-level reform, not only program or service changes.
