What annual reports are required?
Under Chapter 5, Subdivision H of the Aged Care Rules 2025 (sections 166–725, 166–730 and 166–735), providers must submit:
- an AAR
- a Service Demographic Report
- an annual statement of financial compliance and income and expenditure.
How to complete the reports
There is a template available for each report. You will need to provide information on:
- service delivery (aged care services)
- client demographics and service access (via the Service Demographics Report)
- activity and service usage
- workforce and operations
- financial and compliance information (via the Statement of Financial Compliance and Income and Expenditure).

Important note – use of the AAR form
The downloadable Annual Activity Report (AAR) form is provided as an approved data collection tool to support providers in gathering information within their MPS or organisation. It is not an approved submission format. It should be used for preparation purposes only.
The AAR must be submitted via the Qualtrics online reporting platform, which is the approved form under the Aged Care Rules 2025. Completed PDF or Word versions of the AAR will not be accepted as a valid submission.
