Support at Home program

We are reforming in-home aged care. The new Support at Home program replaced the Home Care Packages Program and Short-Term Restorative Care Program on 1 November 2025. The Commonwealth Home Support Program will transition to the new program no earlier than 1 July 2027.

Transitioning to Support at Home

Support at Home has replaced the Home Care Packages (HCP) Program and Short Term-Restorative Care Programme as of 1 November 2025. To ensure a smooth transition, Support at Home providers should complete remaining transitional activities in the initial months.

EOI: Pooled funding trial design workshops

Help us design a pooled funding trial for participants living in group settings, starting in 2026.

We are holding online workshops with Support at Home providers on 10, 11 and 13 November 2025 from 2-3pm AEDT.  If you aren’t available, you can still register to provide written feedback.

You can apply for an aged care assessment online and search for local aged care providers that meet your needs.

Service delivery branch transfer form

This form enables Support at Home registered providers to notify the Secretary, Department of Health, Disability and Ageing, of a request to transfer a service delivery branch from one registered provider to another registered provider.

End-of-Life Pathway form

This form is used to provide evidence of eligibility for the End-of-Life Pathway under the Support at Home program.

Service delivery branch merger form

This form enables Support at Home registered providers to notify the Secretary, Department of Health, Disability and Ageing, of the merger of service delivery branches within the same registered provider.
