Notification of bank account details for an approved pharmacy form
This form is to provide or update your banking details to the department for payments made through claiming for the Pharmaceutical Benefits Scheme.
Authority to authorise pharmacist(s) to sign claim forms on behalf of section 91/91B permission holder form
As permission holder under section 91/91B of the National Health Act 1953, you must complete and submit this form to authorise a pharmacist(s) to sign pharmaceutical benefit claim forms and endorse pharmaceutical benefit prescriptions on your behalf.
Notification of a prescribing-only hospital form
As a state or territory participating in the Pharmaceutical Reforms, complete this form to notify the department of approval of a public hospital to prescribe pharmaceutical benefits in accordance with the applicable Pharmaceutical Reform Agreement.
Application for a medical practitioner to supply pharmaceutical benefits form
Complete this form to apply to the department for approval as a registered medical practitioner to supply pharmaceutical benefits under section 92 of the National Health Act 1953.
Request for ministerial approval to supply pharmaceutical benefits at particular premises form
Complete this form to request ministerial approval to supply pharmaceutical benefits at particular premises under subsection 90A(2) of the National Health Act 1953.
