Register a pharmacy name change

How to tell us about a change of business (trading) name for a Pharmaceutical Benefits Scheme (PBS) approved pharmacy.

What you need to know

If you are an approved pharmacist under section 90 of the National Health Act 1953, you can notify us of a change to the registered business (trading) name of your pharmacy through the PBS Approved Suppliers Portal

If the change of your business trading name also involves a change to your Australian Business Number (ABN) and/or Australian Company Number (ACN), please include supporting documents. 

A change of your business name will not change your approval number. 

If you are also relocating your pharmacy or changing ownership, you will need to submit additional applications for each of these processes. 

What you need to do

To tell us about a change to the registered business (trading) name of your pharmacy: 

  1. Complete the Notification of change of pharmacy registered business (trading) name form.
  2. Provide evidence from the relevant state or territory regulatory authority of the name change.
  3. Submit the form and any related documents through the PBS Approved Suppliers Portal
