Authorise a pharmacist to act for a hospital authority

How to authorise a pharmacist to sign Pharmaceutical Benefits Scheme (PBS) claim forms on behalf of a hospital authority.

What you need to know

A hospital authority approved under section 94 of the National Health Act 1953, can authorise a pharmacist to sign PBS claim forms and endorse pharmaceutical benefit prescriptions on behalf of the hospital authority. 

What you need to do

To authorise a pharmacist to act on behalf of a hospital authority: 

  1. Complete the Authority to authorise pharmacist(s) to sign claim forms on behalf of a hospital authority form.
  2. Submit the form and any related documents through the PBS Approved Suppliers Portal
