About the initiative
The Keeping Australians out of Hospital activity funded research to scale up evidence–based interventions and preventive health activities for people with chronic health conditions.
It aimed to improve quality of life for people with chronic and complex conditions and keep them out of hospital. This research activity continues as part of the Preventive and Public Health Research initiative.
Why it is important
People with chronic diseases are often admitted to hospital over and over again. Hospital admissions are expensive and can expose patients to new problems such as infections. Managing chronic conditions better can reduce hospital admissions.
This initiative funded projects that:
- promote health practices based on research evidence
- build on existing, effective health practices
- target specific health problems
- help community-based health services put research findings into practice
- decrease avoidable hospital admissions
- deliver health services that are good value for money.
Goals
This initiative aimed to:
- reduce avoidable hospitalisations
- help people better manage their chronic and complex health conditions
- promote lifestyle changes that improve health
- support patient care out of hospital, such as timely care at home.
Who we work with
The Medical Research Future Fund (MRFF) continues to fund this initiative through the Preventive and Public Health initiative.
The National Health and Medical Research Council (NHMRC) handled the grant applications.
Status
Keeping Australians out of Hospital has been consolidated into the Preventive and Public Health Research initiative under the MRFF 10-year plan.
