Apply for Support at Home and new Aged Care Act Transition Support 2024-25 Grant Opportunity (GO7421)
This grant opportunity will provide one-off funding of $10,000 (excl GST) towards the cost of upgrading IT systems to meet new reporting obligations under the new Aged Care Act and CHSP providers’ 2025-27 grant agreements.
Apply online via GrantConnect before 2pm (AEDT) 25 February 2025.
CHSP 2025–27 extension
The CHSP is being extended from 1 July 2025 to 30 June 2027.
Ahead of the transition to Support at Home, the introduction of the new Aged Care Act will change the way CHSP services are funded, regulated and delivered.

You can also call 1800 200 422 Monday to Friday, 8 am to 8 pm, and Saturday, 10 am to 2 pm.
Commonwealth Home Support Programme (CHSP) Manual
This 2024–2025 manual provides an overview of the Commonwealth Home Support Programme (CHSP), including the service types it funds and requirements for delivering those services. It is for CHSP-funded service providers to use, and forms part of their CHSP grant agreement.
