Bonded medical schemes – Example statutory declaration
This document shows an example of a completed Commonwealth statutory declaration form. Participants in our bonded medical schemes can use this example to help them fill in a statutory declaration about their return of service obligations.
Bonded Medical Program – Opt in request form
Participants in the Medical Rural Bonded Scholarship (MRBS) Scheme and Bonded Medical Places (BMP) Scheme can use this form to let us know they are interested in opting in to the Bonded Medical Program.

Notice under s19(2) of the Bonded Medical Program Rule – 30 June 2022
For the purposes of section 19(2) of the Health Insurance (Bonded Medical Program) Rule 2020, the Department of Health has, on 30 June 2022, published a notice that the elements of the Department’s web portal required for a bonded participant to comply with each relevant provision are functional.
Bonded Medical Program agreement to participate
Existing participants of the Medical Rural Bonded Scholarship Scheme and Bonded Medical Places Scheme who want to opt in to the Bonded Medical Program should complete the relevant form and email it to us with their request to opt in.
