Accessing care in practice

This case study shows an example of how someone may follow the steps to access funded aged care services.

Case study: Mr Hayward is 74 years old and is living at home. He recently had a fall and is now in hospital. Mr Hayward will need extra support so he can return home. The hospital asks for an aged care assessment. 

The outcomes of Mr Hayward’s assessment reveal several risk factors. His health and functioning are declining, and he has had several falls leading to hospital admissions in recent months. He lives alone, feels socially isolated and needs personal care to help him to keep living independently in his home.

Assessment: Mr Hayward has a needs assessment with an approved needs assessor. He is actively involved in creating his care plan through conversations with his needs assessor. The assessor’s report advises that Mr Hayward needs access to certain funded services, with an urgent priority rating. The assessor recommends certain home support services for personal care and social support, as well as some assistive technology. The delegate reviews the report and approves the care plan. Mr Hayward is given a place within the system.

Outcome: Mr Hayward can safely leave hospital and go back into his home. Mr Hayward quickly receives the appropriate support for his ongoing health. He feels listened to and informed about the process. He is able to keep living in his home independently, while his care plan helps to meet his social and personal care needs.
