Hearing Services Program provider notice – Updated forms

Updated forms for the Hearing Services Program are now available. Providers must use the updated forms from 1 July 2025.

Updated versions of program forms are now available.

The updated forms are the final piece of the program web transition, which involved the new website at health.gov.au/hear and update of content and resources. Improvements have been made to accessibility and usability and you can complete all forms digitally. The forms have been updated to improve readability and give greater clarity for clients completing the forms.

There is space at the top of the forms for providers to add branding. Providers can adapt the design of the forms for their purposes however you cannot alter the text.

Providers must use the updated forms from 1 July 2025. Please ensure you update your systems to use the new forms from this date. You may use the updated forms sooner if this suits your business.
