Have your say on the Home Care Package Program Activity Survey

We want to hear from all Home Care Package (HCP) providers to better understand the volume of their current service delivery.

Home Care Package (HPC) providers are transitioning to the Support at Home program funding model from 1 July 2025.

To help us support HCP providers to transition to Support at Home, we are collecting data on service hours and units delivered to care recipients by service type. 

If you are a HCP provider, please complete the Home Care Package Program Activity Survey by 1 July 2024. 

We will use your NAPS ID to avoid collecting already known information, like location and organisation size.

This survey is not an audit of individual provider capacity and we will not use for any compliance action. 

Providers who are members with ACCPA can opt-in to share their data with them.
