Schedule

Stoma Appliance Scheme

The Stoma Appliance Scheme provides free stoma appliances to people who have a stoma (ostomates), through their stoma associations. We list all the items subsidised under the scheme on the Stoma Appliance Scheme Schedule.

The Stoma Appliance Scheme schedule lists all of the stoma-related products and appliances that we subsidise under the scheme. You can view the full list, or just the additions, deletions or variations.

Forms for ostomates

Stoma Appliance Scheme – Deodorant and absorption gelling sachets authorisation form

Stomal therapy nurses should sign this form to authorise a patient who requires it to order deodorant and absorption gelling sachets from their stoma association.

Stoma Appliance Scheme – Tieman tip catheter authorisation form

Stomal therapy nurses should sign this form to authorise a patient who requires a Tieman tip catheter to order it from their stoma association.

Stoma Appliance Scheme – Irrigation kit authorisation form

Stomal therapy nurses should sign this form to authorise a patient who requires an irrigation kit to order it from their stoma association.

Stoma Appliance Scheme Australian Privacy Principle 5 notification and consent form

Sign this form if you are an ostomates who needs more than 4 times the maximum quantity allowed for stoma products and appliances listed in the Stoma Appliance Scheme Schedule. This allows us to access personal information so that we can assess your eligibility.
