Aged care programs for the health sector

Support at Home

In-home aged care has changed to better help older people live independently at home for longer. The new program replaces the Home Care Packages Program and Short-Term Restorative Care Programme.

Commonwealth Home Support Programme (CHSP)

Support for older people who need some help to stay at home and maintain independence. Support can include help with daily tasks, home modifications, transport, social support and nursing care.

Residential aged care

If an older person is no longer able to live independently at home, we fund residential aged care to make it more affordable and accessible. This includes accommodation and personal care 24 hours a day.

The detailed Integrated Assessment Tool (IAT) classification algorithm decision tree shows how a combination of functional ability score, needs met score and compounding factors provides Commonwealth Support at Home Program (CHSP) and Support at Home program classification categories.

The report provides an overview of demographic data including demand, supply, entry and departures from the system.

Support at Home classification system

A standardised set of information is entered into the Integrated Assessment Tool (IAT) by an assessor is used to evaluate an older person's functional ability, needs met and other relevant factors. This guideline provides information on how the algorithm is guided to generate a classification level.
