Support at Home’s short-term pathways

Support at Home offers both ongoing services and 3 short-term pathways. This video explains each pathway and how they work alongside, or instead of, ongoing services.

The Support at Home program makes it easier for older people to stay at home for longer. 

 
Support at Home has ongoing services and three short-term pathways.

 
Ongoing services are regular care and support with daily activities, like nursing, showering and meal preparation.

 
The short-term pathways are the Restorative Care Pathway, the End-of-Life Pathway and the Assistive Technology and Home Modifications scheme.

 
The short-term pathways work alongside, or instead of, ongoing supported home services.

 
The Restorative Care Pathway offers short-term access to allied health and nursing support to help you regain strength and confidence after illness or injury.

 
If you already receive ongoing services, you can access this pathway at the same time. But if you're new to in home care, you can only be approved for this pathway or ongoing services.

 
The End-of-Life Pathway supports older people with 3 months or less to live and who wish to remain at home – surrounded by loved ones and familiar comforts.

 
It works alongside state-funded palliative care, providing urgent funding for personal care, help around the house and nursing.

 
This pathway replaces ongoing Support at Home services.

 
The Assistive Technology and Home Modification Scheme funds equipment and home upgrades, like ramps and grab rails, to meet your assessed needs. To help make your home safer and easier to move around in.

 
You can access this funding on top of ongoing services and the other pathways.
