Important information
There is formatting in the date fields of the bulk submission template. When pasting data into the bulk submission template from another source, use the ‘paste values’ function to keep the format.
Delete all previous versions of this template and only use the appropriate template available on the QI Program resources page.

Template – Approved provider portal – up to and including Quarter 1 FY 2025–26
Use for submitting and updating QI Program data up to and including Q1 Financial Year (FY) 2025–26. Also use for any approved providers who are operational in October 2025, who will cease operations before 1 November 2025 (to report Q2 FY 2025–26 QI Program data).
