Freedom of information (FOI) application form

To apply for access to our documents under the FOI Act, you will need to provide:

• your details
• a clear description of the documents you want to access
• your preferred way of accessing the documents, such as by email.

If you are applying on behalf of another person, you will need to provide:

• the details of the applicant
• evidence that you have authorisation, such as a signed letter.
